BUILDING REPAIRS DUE TO HURRICANE IDA DAMAGE

The City of Mandeville is requesting quotes from General Contractors to furnish all labor, equipment, and materials
necessary for building repairs at various city locations due to damage resulting from the landfall of Hurricane
Ida.

All applicable laws, ordinances, and the rules and regulations of all authorities having jurisdiction over this purchase
shall apply to the contract throughout. All regulations and policies applicable to the FEMA Public Assistance program
shall be included in the contract. The contractor must provide proof of insurance meeting or exceeding the enclosed
City of Mandeville Insurance Requirements prior to contract execution. There is NO BID BOND requirement for
this solicitation.

Please include the referenced project name and the contractor’s name, address, and LA State Contractor’s License
Number on the quotation. Contractors are to provide Lump Sum pricing based on given quantities and contractor
provided unit costs. Final project costs will be based on actual quantities installed on the project. Contractors need to
make sure they provide all unit costs outlined on the unit cost bid form.
